  1. A description of who you are (i.e. consumer or healthcare professional)
  2. Your name and contact information
  3. A detailed description of the product
  4. Name of the manufacturer or importer of the product
  5. Date the incident occurred
  6. Details of what happened (type of injury/death or type of risk of injury/death from product)
  7. Permission to publish your report on SaferProducts.gov
  8. Verification that all of the details provided are accurate to the best of your memory and belief
